ATHLETIC PARTICIPATION FOR THE 2018-2019 SCHOOL YEAR

Participating students and their parents are required to read the PSD ATHLETIC HANDBOOK .

A Permit to Practice Card must be obtained each season before tryouts.  It will be issued after the following forms are completed  (total of 4 forms - 5 pages to fill out) and proof of payment is presented.  Cards for fall sports will begin to be issued July 25th.  Forms can be found on the following website:  https://www.psdschools.org/school-resources/athletics/secondary-school-sports , or below.  For  Athletic Fees see info below:

2018-19 Forms

  1. 2018-2019 Physician Certification of Student Fitness for Athletic Participation   (Español).   IMPORTANT - Doctor signs and prints his name on the bottom of page, and, indicates "Date of Examination."  Physical is good for one year.
  2. 2018 - 2019 HS Athletic Participation & Release Form  (Espanol must be provided every new school year so we have the most current insurance info/signatures!  Be sure to complete the front and back of this page.
  3. 2018-2019 Student Athlete Medical Information Form     (Español)  - must be provided every new school year!  This form goes to our trainer and info must be current.  If athlete carries/or self medicates, following forms must be on file with the school nurse!!!

  4.  2018-2019 CHSAA Eligibility Form    (Español)  - must be provided every new school year!

  5. Pay athletic fee. Payment must be made before "Permit to Practice" card is issued!  Students who do not pay will not be allowed to participate!  How do I pay?   There are 2 ways to pay.  1) in person at RMHS Fee window, 2) pay online at https://psdschools.schoolpay.com with either debit or credit card (3.35% transaction fee charged).  Registration is easy.

    High School Fees: Please note there is NOT a family maximum.

    PSD Non-Charter Students:
    All Sports (except ice hockey and lacrosse): $120 per sport
    ​Boys & Girls Lacrosse: $175 plus $120
    Boys Ice Hockey: $1,000 plus $120

    Home-Based, Charter and Private School Students:
    All Sports (except ice hockey and lacrosse): $180 per sport
    Boys & Girls Lacrosse: $266 plus $180
    Boys Ice Hockey: $1,000 plus $180

    High School Unified Sports 
    $65.00 per sport for PSD Non-Charter Students
    $98 per sport for Home-Based, Charter & Private School Students

  6. Athletic Fitness Center Donation - $5.00 (one time amount to support maintenance and equipment replacement in the Fitness Center.

Forms must be turned into the Athletics Secretary. Athletic Secretary will issue a "Permit to Practice" card to the athlete. Athletes will give the permit to the coach; this indicates to the coach that all of the athlete's paperwork and payment is complete.

When will orange "Permit to Practice" cards be issued??

"Permit to Practice" cards will not be issued for the fall season starting July 15th!
If athlete is not participating until Winter season...."permit to practice" cards will begin to be issued in October.
If athlete is not participating until Spring..."permit to practice" cards will begin to be issued in January, once grades have been recorded for the first semester.

*Indicates athletic programs that are not offered by all of the four comprehensive high schools.  Instead the programs are combined teams. CHSAA requires the combined teams to be assigned to a specific high school.  The information below identifies which high school where these combined sports are offered. Students interested in the combined athletic programs may register at assigned school or through the athletic office at their high school (if a student at one of the four PSD comprehensive high schools).

Sports Physicals Offered at the Health and Wellness Center at Centennial High School

For the past two years we have offered Summer sports physicals/well child exam clinics and we will be open again this summer!  Please see the announcement below and  share it in school newletters, mailings, one on one with students, etc...  We have a new website (hwcenter.org) coming soon (end of May/beginning of June), so please check it out!

Do you know a student that needs a Sports Physical/Well Child exam?   The Health and Wellness Center at Centennial High School is now able to enroll and see any PSD Middle or High School student (and their PSD enrolled siblings).  We prioritize meeting the needs of students/families that are uninsured or underinsured and/or having a difficult time accessing medical care.   The Center can and does bill insurance, including Medicaid and CHP+.  We also offer a cash pay option and a sliding fee scale for qualifying families.  No student is ever turned away for inability to pay!

To be seen, students/families MUST make an appointment by calling the Center's main number at 970.488.4950 to schedule an appointment and receive enrollment and consent forms.  Summer appointments will be available June 26 - 29, July 17 - 31 and August 1- 18.  Appointment times are limited and generally fill quickly so scheduling as early as possible is highly recommended!

Transfer Athletes 

We ask that all students who are transferring in from another school, or, district, to fill-out forms 9 from the Colorado High School Activities Association. The athlete must be enrolled at Rocky in order to be accepted!  See links below too access documents. 

After starting high school, any time a student moves from one school to another school the student is considered a "transfer" student!!  The following paperwork must be filled out.  Please read the following handbook regarding transfers:  Transfer Waiver and Parent Handbook.

FOREIGN EXCHANGE ATHLETES

This form is to be used to obtain eligibility for all foreign students who are at a school with no bona fide family move. This includes students who are and are not on approved exchange programs - two pages.

ATHLETIC REQUIREMENT FOR CHARTER SCHOOL/HOMESCHOOL ATHLETES

If a new athlete is not enrolled at Rocky and is a charter/homeschool student, they must first get clearance by the District Athletic Director - Russ McKinstry. Once the Athletic Director has cleared an athlete to participate at one of the four high schools, the athlete's family and the high school will receive a placement letter.  Contact info for Russ McKinstry's Office/Secretary is Lori Willadsen, 970-490-3332 or e-mail at lwilladsen@psdschools.org.  Please access following site for more info:  https://www.psdschools.org/placement-athletics.

REFUND POLICY AND CONDITIONS FOR REFUNDS
 

All athletic fee refunds for cut/drop will be approved by the athletic director at each school.

Once the Athletic Department receives confirmation from the coach that an athlete is no longer participating, refunds can be made.

If the athletic fee(s) were previously waived due to family maximum or free/reduced lunch eligibility, there may be no refund or partial refund payment amount due.  Check the student's receipt before you request a refund for cut/drop.

Students will be eligible for a full or partial refund of athletic fees due to cut/drop only under the following conditions. If sport was paid via credit card, refund will be returned the same way with additional credit card fees taken out.
